Responsibilities
- Coordinate patient care plans across multidisciplinary teams
- Manage electronic health records (EHR) with 99% accuracy
- Facilitate smooth patient transitions between care settings
- Conduct regular patient assessments and care reviews
- Maintain HIPAA compliance in all documentation
- Collaborate with insurance providers for authorization processes
- Train new staff on department protocols
Qualifications
- Associate's degree in Healthcare or related field (or equivalent experience)
- Minimum 2 years of clinical coordination experience
- Certified Medical Assistant (CMA) or RHIT preferred
- Proficiency in Epic EHR systems
- Exceptional communication and interpersonal skills
- Ability to work flexible hours including weekends
- Strong problem-solving abilities in high-pressure situations
- Valid Maryland healthcare provider license
