Responsibilities
- Coordinate patient care plans across multidisciplinary healthcare teams
- Manage patient intake and scheduling with precision and empathy
- Facilitate community health outreach initiatives in Omaha
- Maintain accurate electronic health records and documentation
- Collaborate with social services to address patient needs holistically
- Train new staff on healthcare protocols and patient engagement

Qualifications
- Bachelor's degree in Healthcare Administration or related field
- 2+ years of clinical or healthcare coordination experience
- Strong proficiency with EHR systems (Epic preferred)
- Excellent communication and conflict resolution skills
- Nebraska healthcare license or ability to obtain immediately
- Ability to work flexible hours including evenings/weekends
- Certification in Healthcare Management (CHM) preferred
