Responsibilities
- Provide direct patient care including vital signs, wound care, and medication administration
- Assist patients with daily living activities and mobility support
- Document patient observations and care plans accurately in electronic health records
- Collaborate with interdisciplinary healthcare teams to ensure coordinated care
- Maintain a clean, safe, and organized patient environment
- Educate patients and families on health management protocols
- Adhere to HIPAA regulations and infection control standards
Qualifications
- High school diploma or equivalent (CNA certification preferred)
- Minimum 1 year of healthcare or direct patient care experience
- Current CPR/BLS certification
- Strong communication and interpersonal skills
- Ability to stand/walk for extended periods and lift up to 50 lbs
- Proficiency in electronic health record systems
- Background check and drug screen required
- Valid Illinois driver's license (if transportation duties assigned)
