Responsibilities
- Coordinate patient care plans and schedules across multidisciplinary teams
- Facilitate seamless communication between patients, families, and healthcare providers
- Manage electronic health records (EHR) systems with precision and confidentiality
- Conduct patient intake assessments and eligibility verifications
- Collaborate with social workers to address patient needs and resources
- Monitor and report patient outcomes to clinical supervisors
- Participate in continuous improvement initiatives for care delivery
Qualifications
- Associate's degree in healthcare administration or related field (Bachelor's preferred)
- 2+ years of experience in healthcare coordination or similar role
- Proficiency with EHR systems (Epic or Cerner certification a plus)
- Strong communication and interpersonal skills
- Ability to work independently and prioritize tasks in high-pressure situations
- Valid Indiana state healthcare license or certification
- Exceptional attention to detail and organizational abilities
- Flexibility to work occasional evenings/weekends as needed
