Responsibilities
- Coordinate patient care plans across multidisciplinary teams
- Manage electronic health records (EHR) with precision and confidentiality
- Facilitate seamless transitions between care settings (hospitals, clinics, home care)
- Conduct patient intake assessments and eligibility verifications
- Collaborate with insurance providers for authorization and billing
- Maintain compliance with HIPAA and healthcare regulations

Qualifications
- Minimum 2 years of healthcare coordination or clinical support experience
- Certified Medical Assistant (CMA) or Licensed Practical Nurse (LPN) preferred
- Proficiency in EHR systems (Epic, Cerner, or similar)
- Strong knowledge of healthcare regulations and compliance standards
- Exceptional communication and interpersonal skills

- Valid Texas healthcare professional license or certification
